Introduction: Current literature reports the effectiveness of myofunctional therapy in synergy between speech therapy and orthodontics from an holistic perspective. Both professional fields are concerned with the health of the stomatognathic apparatus and the oral functions within it. This connection allows us to understand how synergetic work can achieve optimal results that are satisfactory for both professions. Working hypothesis: The study aims to highlight the close relationship between the two disciplines. The aim of our work is to compare speech therapy assessment with orthodontic assessment in childhood in order to set up a combined ad hoc intervention specific to the patient's needs. Materials and methods: The study was carried out in a private dental practice. The sample consisted of 117 children of developmental age between 2 and 14 years. For the anamnestic collection, a questionnaire was prepared for the parents with a focus on the children's oral behaviour and habits. Together with the questionnaire, an information leaflet on myofunctional therapy was provided to the parents. For the interdisciplinary assessment, an integrated tool was prepared between the orthodontist and speech therapist: 'Speech Therapist-Orthodontist Referral and Sharing Protocol (P.I.C.L.O.), (2021) modified by adding the assessment of the sinovio-temporo-mandibular joint (AsTM). Results: The study reported a high incidence of oral dysfunction in the evaluated patients, even in subjects in whom malocclusion correction was performed. A close correlation is also observed between specific dysfunctions (such as oral breathing, dysfunctional swallowing and mastication disorders) and particular occlusal-dental patterns (Class II, increased overjet, open bite, crowding). In addition, a discrepancy was found between the clinical assessment and the parents' perception. Conclusions: The study confirms the close relationship between oral dysfunction and occluso-dental alterations, underlining the need for interdisciplinary care between speech therapy and orthodontics. Integrated assessment makes it possible to intercept oral dysfunctions at an early stage and consequently improve treatment, setting up more effective and lasting interventions. Systematic screening and educational paths for caregivers should be increased in order to increase preventive and therapeutic effectiveness.
Introduzione: la letteratura attuale riporta l’efficacia della terapia miofunzionale in sinergia tra logopedia e ortodonzia in ottica olistica. Entrambe le branche professionali si occupano della salute dell’apparato stomatognatico e delle funzioni orali che in esso si esplicano. Tale connessione permette di comprendere come il lavoro sinergico permette di ottenere risultati ottimali soddisfacenti per entrambe le professioni. Ipotesi di lavoro: Lo studio vuole evidenziare la stretta relazione tra le due discipline. Lo scopo del nostro lavoro è quello di comparare la valutazione logopedica con quella ortodontica in età evolutiva al fine di impostare un intervento ad hoc combinato e specifico per il bisogno del paziente. Materiali e metodi: Lo studio è stato svolto presso uno studio odontoiatrico privato. Il campione è costituito da 117 bambini in età evolutiva compresa tra i 2 e i 14 anni. Per la raccolta anamnestica si è predisposto un questionario rivolto ai genitori con particolare attenzione ai comportamenti e abitudini orali dei bambini. Assieme al questionario è stato fornito ai genitori un dépliant informativo relativo alla terapia miofunzionale. Per la valutazione interdisciplinare si è predisposto uno strumento integrato tra ortodontista e logopedista: ‘Protocollo di Invio e Condivisione Logopedista-Ortodontista (P.I.C.L.O.), (2021) modificato attraverso l’aggiunta della valutazione dell’articolazione sinovio-temporo-mandibolare (AsTM). Risultati: Lo studio ha riportato un’elevata incidenza di disfunzioni orali nei pazienti valutati, anche in soggetti in cui è stata effettuata correzione della malocclusione. Si osserva inoltre una stretta correlazione tra specifiche disfunzioni (come respirazione orale, deglutizione disfunzionale e disordini della masticazione) e particolari pattern occluso-dentali (Classe II, overjet aumentato, morso aperto, affollamento). Inoltre è stata rilevata una discrepanza tra la valutazione clinica e la percezione dei genitori. Conclusioni: Lo studio conferma la stretta relazione tra disfunzioni orali e alterazioni occluso dentali, sottolineando la necessità di una presa in carico interdisciplinare tra logopedia e ortodonzia. La valutazione integrata permette di intercettare precocemente i disordini oromiofunzionali e di conseguenza di migliorare il trattamento, impostando interventi più efficaci e duraturi. È auspicabile incrementare gli screening sistematici e i percorsi educativi per i caregiver affinché aumenti l'efficacia preventiva e terapeutica.
