The topic of end of life is difficult to address. In this regard, Monica Vaccaretti explains that: "Even just talking about it unconsciously creates discomfort that we try to push away with various defense mechanisms." For a greater analysis of the topic, the concept of Compassion Fatigue is proposed, developed by Doctor Charles R. Figley between 1985 and 1992, which refers to a feeling of chronic stress, emotional exhaustion and tension often felt by those who work in the helping professions as a reaction to the great suffering of those they help. This emotional burden is also recognized by Lauro (2018), who states that "It is a clinical condition that makes it impossible to establish an effective therapeutic relationship". For this reason, some health workers tend to adopt avoidance attitudes, as they do not know how to deal with such an intimate and delicate moment for the person and their relatives. Being able to identify the right skills to support the patient and his family, in this complex journey, can prove difficult. One of the solutions proposed by the literature is the SPIKES protocol (2017), among whose objectives we find that of also helping the nurse to support the patient in the choices regarding the end-of-life path, providing the healthcare professional with six useful phases to help communication in similar moments.
Il tema del fine vita è difficile da affrontare. A tal proposito, Monica Vaccaretti spiega che: "Anche solo parlarne crea inconsciamente un disagio che cerchiamo di allontanare con vari meccanismi di difesa". Per una maggiore analisi dell'argomento, viene proposto il concetto di Compassion Fatigue, sviluppato dal dottor Charles R. Figley tra il 1985 e il 1992, che si riferisce a una sensazione di stress cronico, esaurimento emotivo e tensione, spesso avvertita da chi lavora nelle professioni d'aiuto come reazione alla grande sofferenza di chi aiuta. Questo carico emotivo è riconosciuto anche da Lauro (2018), il quale afferma che "è una condizione clinica che rende impossibile stabilire una relazione terapeutica efficace". Per questo motivo, alcuni operatori sanitari tendono ad adottare atteggiamenti di evitamento, in quanto non sanno come affrontare un momento così intimo e delicato per la persona e i suoi familiari. Essere in grado di individuare le giuste competenze per supportare il paziente e la sua famiglia, in questo complesso percorso, può rivelarsi difficile. Una delle soluzioni proposte dalla letteratura è il protocollo SPIKES (2017), tra i cui obiettivi troviamo quello di aiutare anche l'infermiere a supportare il paziente nelle scelte riguardanti il percorso di fine vita, fornendo all'operatore sanitario sei fasi utili per aiutare la comunicazione in momenti simili.
