Hypersexuality is a complex clinical condition, still lacking a shared diagnostic framework and often underestimated in clinical practice. It may arise secondarily to various neurological disorders, with heterogeneous clinical and etiopathogenetic features. This thesis aims to systematically explore the manifestations of hypersexuality in neurological disorders, with the objective of investigating the main etiopathogenetic hypotheses and critically analyzing the intervention strategies documented in the literature. To this end, a systematic literature review was conducted according to PRISMA 2020 guidelines. Descriptive, observational, and experimental studies were included, selected through searches in five databases: PubMed, PsycINFO, Scopus, Web of Science, and CINAHL. The methodological quality of the studies was assessed using validated tools, selected according to each study’s design. The review revealed that hypersexuality is a heterogeneous phenomenon, whose manifestations vary depending on the underlying neurological disorder and are influenced by pharmacological treatments, neurodegenerative processes, psychiatric comorbidities, and individual vulnerabilities. Three cross-cutting critical issues emerged: the absence of specific assessment tools, the lack of shared clinical guidelines, and the risk of underdiagnosis. The results highlight the need for greater diagnostic and methodological rigor, especially considering the relational, ethical, and forensic implications of the disorder. This thesis aims to contribute, at least in part, to bridging these gaps by offering a critical overview of the available evidence, identifying underexplored areas, and promoting the adoption of an integrated therapeutic approach that considers the complexity of hypersexuality across different neurological contexts.
L’ipersessualità è una condizione clinica complessa, ancora priva di un accordo diagnostico condiviso e spesso sottovalutata nella pratica clinica. Può manifestarsi secondariamente a diversi disturbi neurologici, con caratteristiche cliniche ed eziopatogenetiche fortemente eterogenee. La presente tesi si propone di esplorare in modo sistematico le manifestazioni dell’ipersessualità nei disturbi neurologici, con l’obiettivo di approfondirne le principali ipotesi eziopatogenetiche e analizzare criticamente le strategie di intervento documentate in letteratura. A tal fine, è stata condotta una revisione sistematica della letteratura secondo le linee guida PRISMA 2020 Sono stati inclusi studi descrittivi, osservazionali e sperimentali, selezionati tramite consultazione di cinque banche dati: PubMed, PsycINFO, Scopus, Web of Sciance e CINAHL. La qualità metodologica degli studi è stata valutata mediante strumenti validati, scelti in base al disegno metodologico di ciascun articolo. La revisione ha evidenziato che l’ipersessualità è un fenomeno eterogeneo, le cui manifestazioni variano in relazione al disturbo neurologico sottostante, risultando influenzate da terapie farmacologiche, processi neurodegenerativi, comorbidità psichiatriche e vulnerabilità individuali. Sono emerse tre criticità trasversali: l’assenza di strumenti di valutazione specifici, la mancanza di linee guida condivise e il rischio di sottodiagnosi. I risultati evidenziano la necessità di un maggiore rigore diagnostico e metodologico, anche in relazione alle implicazioni relazionali, etiche e forensi del disturbo. La tesi intende contribuire a colmare, almeno in parte, queste lacune, offrendo una panoramica critica delle evidenze disponibili, individuando le aree ancora poco esplorate e promuovendo l’adozione di un approccio terapeutico integrato, che consideri la complessità dell’ipersessualità nei diversi contesti neurologici.
