Writing this thesis represents the conclusion of my educational journey and, at the same time, the beginning of a deeper reflection on what it means to work in the field of mental health. Adolescent mental health is today one of the most urgent and complex challenges for welfare systems. The rise in psychiatric diagnoses among young people, the evolving needs of individuals and families, and the social transformations of recent decades call for an integrated approach—one that brings together healthcare, psychological, educational, and social expertise. In this context, the role of social work emerges as a cornerstone in building and maintaining the care network, especially during critical moments of service transition, such as the shift from Child and Adolescent Neuropsychiatry to Adult Psychiatry. This thesis is structured into four chapters. The first chapter provides a theoretical and regulatory framework, outlining the mental health service system with a specific focus on the transition from Child and Adolescent Neuropsychiatry to Adult Psychiatry. The second chapter explores the role of the social worker within these services, highlighting their strategic functions in mediating between institutions and individuals, and in safeguarding rights throughout care pathways. The third chapter addresses the issue of transition, examining its main challenges and outlining possible responses through individualized support and personalized care planning. Finally, the fourth chapter presents the case of S., a young person followed during my internship. Through this case, the relational dynamics, the tools used by social services, and the reflections developed in the field experience are illustrated, with particular attention to the continuity of care between NPI and Psychiatry.
Scrivere questa tesi rappresenta per l’autrice la conclusione di un percorso formativo e, al tempo stesso, l’inizio di una riflessione più profonda su cosa significhi operare nel campo della salute mentale. La salute mentale in adolescenza costituisce oggi una delle sfide più urgenti e complesse per i sistemi di welfare. L’aumento delle diagnosi psichiatriche tra i giovani, l’evoluzione dei bisogni individuali e familiari, insieme alle trasformazioni sociali degli ultimi decenni, impongono un approccio integrato, capace di coniugare competenze sanitarie, psicologiche, educative e sociali. In questo scenario, il ruolo del servizio sociale emerge come elemento cardine nella costruzione e nel mantenimento della rete di cura, in particolare nei momenti delicati di transizione tra servizi, come il passaggio dalla Neuropsichiatria Infantile alla Psichiatria per adulti. La tesi si articola in quattro capitoli. Il primo capitolo propone una cornice teorica e normativa, ricostruendo il sistema dei servizi di salute mentale con un focus specifico sul passaggio di competenze tra Neuropsichiatria Infantile e Psichiatria. Il secondo capitolo approfondisce il ruolo dell’assistente sociale all’interno di tali servizi, evidenziandone le funzioni strategiche nella mediazione tra istituzioni e persone, nonché nella tutela dei diritti nei percorsi di cura. Il terzo capitolo è dedicato al tema della transizione, analizzandone le principali criticità e delineando possibili risposte attraverso strumenti di accompagnamento individualizzato e progettazione personalizzata. Infine, il quarto capitolo presenta il caso di S., un ragazzo seguito durante il tirocinio, attraverso il quale vengono illustrate le dinamiche relazionali, gli strumenti utilizzati dal servizio sociale e le riflessioni sviluppate nel corso dell’esperienza sul campo, con particolare attenzione alla continuità della presa in carico tra NPI e Psichiatria.
