This thesis examines the role of public intervention in the field of healthcare innovation, with a particular focus on innovative drugs and medical devices. The first chapter explores the role of the State in the healthcare sector, referring both to the Italian context, through an analysis of the National Health Service, and to the European context, by investigating regulatory instruments and public funding mechanisms for research. The second chapter delves into the biomedical innovation process, distinguishing the various phases of research related to drugs and medical devices, and analyzing the role of public and private actors, as well as emerging technologies such as artificial intelligence. The final chapter focuses on concrete cases of innovative drugs and devices, offering an analysis of the challenges related to their accessibility and sustainability, and reflecting on their economic impact on the healthcare market. The analysis highlights how healthcare innovation requires a balance between public intervention and private investment, supported by effective regulation, in order to ensure progress without compromising the equity and sustainability of the healthcare system.
La presente tesi analizza il ruolo dell'intervento pubblico nell'ambito dell'innovazione sanitaria, ponendo un focus sui farmaci e dispositivi medici innovativi. Nel primo capitolo viene esaminato il ruolo dello Stato nel settore sanitario con riferimento sia al contesto italiano, tramite un'analisi del Servizio Sanitario Nazionale, sia al contesto europeo, attraverso una ricerca riguardante gli strumenti di regolazione e i meccanismi di finanziamento pubblico della ricerca. Il secondo capitolo approfondisce il processo di innovazione in campo bio-medico, distinguendo le diverse fasi della ricerca dei farmaci e dispostivi medici, e il ruolo dei soggetti pubblici, privati e delle nuove tecnologie come l'intelligenza artificiale. L'ultimo capitolo si concentra su alcuni casi concreti di farmaci e dispositivi innovativi con un'analisi delle criticità relative alla loro accessibilità e sostenibilità e con una riflessione sugli impatti economici che producono sul mercato sanitario. Da questa analisi è emerso come l'innovazione sanitaria richiede un equilibrio tra intervento pubblico e investimenti privati, uniti a una regolamentazione efficace, al fine di garantire il progresso senza compromettere l'equità e la sostenibilità del sistema sanitario.
