BACKGROUND. In nursing, the patient-nurse relationship is a cornerstone of the treatment process, contributing to psychophysical well-being and the quality of care provided. An effective therapeutic relationship, based on empathy and communication, facilitates patient adherence and improves clinical outcomes. However, certain dysfunctional communication dynamics can arise that compromise the quality of the relationship, such as the emerging phenomenon of Toxic Positivity, which promotes an overly positive attitude while neglecting the patient’s genuine negative emotions. OBJECTIVES. This review aims to analyze the risks that Toxic Positivity poses to the healthcare profession and to identify strategies that promote respect and authenticity of the patient’s experiences. METHODS. A literature review was conducted by consulting multiple databases: SCOPUS, PUBMED and CINAHL (EBESCO). RESULTS. The selection includes 14 articles that met the inclusion and exclusion criteria: 6 systematic reviews, 1 randomized controlled trial (RCT), 6 qualitative studies and 1 meta-analysis. CONCLUSION.Toxic Positivity can pose serious risks to the nurse-patient relationship and the patients psychological well-being. Among the main risks are impaired trust, emotional invalidation, and the creation of internal dissonance between expressed and authentic emotions, which contributes to undermining the development of resilience in patients throughout the treatment process. The review identified communication and relational strategies aimed at counteracting these negative effects by promoting emotional authenticity, hope, and resilience. These include positive emotion regulation, emotional support from others and emotional validation. Although Toxic Positivity is a recent and still underexplored phenomenon in healthcare, this review highlights the need to further explore its effects and strategies to counteract it, in order to ensure nursing care that focuses on the person as a whole, emotionally and relationally. INTRODUZIONE. Nell’assistenza infermieristica, la relazione tra paziente e infermiere rappresenta un elemento cardine del percorso di cura, contribuendo in modo significativo al benessere psico-fisico e alla qualità delle cure erogate. Una relazione terapeutica efficace, basata sull’empatia e la comunicazione, facilita l’aderenza del paziente e migliora gli esiti clinici. Tuttavia, possono manifestarsi dinamiche comunicative disfunzionali che compromettono la qualità della relazione, come il fenomeno emergente della Positività Tossica, che promuove un atteggiamento eccessivamente positivo trascurando le emozioni negative autentiche del paziente. OBIETTIVO. Descrivere i rischi che la Positività Tossica comporta per la professione sanitaria e individuare strategie che favoriscano il rispetto dell’autenticità e la soggettività. METODI. È stata redatta una revisione della letteratura consultando le banche dati Scopus, Pubmed e Cinahl (EBESCO). RISULTATI. Sono stati selezionati 14 articoli che rispondevano ai criteri di inclusione ed esclusione: di cui 6 revisioni sistematiche, 1 trial controllato randomizzato (RCT), 6 studi qualitativi e 1 meta-analisi. CONCLUSIONI. La Positività Tossica può comportare gravi rischi per la relazione infermiere-paziente e il benessere psicologico del paziente. Tra i principali vi sono la compromissione della fiducia, l’invalidazione emotiva, la creazione di una dissonanza interiore tra emozioni espresse ed autentiche, che contribuisce ad intaccare lo sviluppo della resilienza nel paziente durante il percorso di cura. Strategie comunicative e relazionali mirate possono contrastare tali effetti negativi, promuovendo l’autenticità emotiva, la speranza e la resilienza. Queste includono la regolazione positiva delle emozioni, il supporto emotivo da parte di terzi a e la validazione emotiva. Sebbene la Positività Tossica sia un fenomeno recente e ancora poco esplorato in ambito sanitario, questa revisione evidenzia la necessità di approfondirne ulteriormente gli effetti e le strategie di contrasto, al fine di garantire un’assistenza infermieristica incentrata sulla persona nella sua globalità emotiva e relazionale.
