Introduction: Therapeutic adherence represents a crucial challenge for public health, especially among discharged or non-hospitalized patients who must independently manage their medication therapy and monitor vital signs. According to the European Charter of Adherence to Therapy, drafted by the Senior International Health Association (SIHA, 2016), non-adherence is responsible for approximately 200,000 premature deaths annually in Europe and costs the Italian National Health Service about €2 billion per year (AIFA, Comunicato stampa N. 23/2024). The issue was the subject of an awareness campaign broadcast on major Italian national television networks in March 2025 (Fondazione Pubblicità Progresso & Servier Italia, 2025). Mobile technologies, particularly medication reminder apps, offer new opportunities to address this problem. Objective: The aim of this literature review is to gather the most recent evidence regarding the benefits and challenges of using mobile applications for medication reminders and self- management of health in discharged patients, assessing their impact on therapeutic adherence, reduction of medication errors, and patient empowerment. Methods: A literature review was conducted by consulting the biomedical database “PubMed.” Articles published in the last four years were considered. The selected articles were analyzed based on their relevance, research methodology, and results. Results: Evidence shows that the use of mobile apps for medication reminders significantly improves therapy adherence, reduces medication errors, and supports the monitoring of vital signs. Furthermore, these technologies promote patient autonomy and can be effectively integrated into the hospital discharge process through educational interventions and updates to discharge checklists. Conclusions: Mobile applications represent a promising tool to support patients in managing post-discharge therapy. Integrating these technologies into discharge protocols, accompanied by targeted educational interventions, can help improve therapeutic adherence and reduce the risks associated with self-management of health. Introduzione: L’aderenza terapeutica rappresenta una sfida cruciale per la sanità pubblica, soprattutto nei pazienti dimessi o non ricoverati, che devono gestire autonomamente la propria terapia farmacologica e il monitoraggio dei parametri vitali. La mancata aderenza è responsabile, secondo la Carta Europea di aderenza alla terapia, redatta dal Senior International Health Association (SIHA, 2016), di circa 200.000 decessi prematuri annui in Europa, e di un costo per il Servizio Sanitario Nazionale Italiano di circa 2 miliardi di euro l’anno (AIFA, Comunicato stampa N.23/2024). Il tema è stato oggetto di una campagna di sensibilizzazione trasmessa dalle principali reti televisive nazionali italiane nel marzo 2025 (Fondazione Pubblicità Progresso & Servier Italia, 2025). Le tecnologie mobili, in particolare le app per il promemoria terapeutico, offrono nuove opportunità per affrontare questo problema. Obiettivo: L’obiettivo di questa revisione di letteratura è reperire le evidenze più recenti riguardanti i vantaggi e le criticità dell’utilizzo di applicazioni mobili per il promemoria terapeutico e la gestione autonoma della salute nei pazienti dimessi, valutando il loro impatto sull’aderenza terapeutica, sulla riduzione degli errori di assunzione e sull’empowerment del paziente. Metodi: È stata condotta una revisione della letteratura scientifica tramite consultazione della banca dati in ambito biomedico “PubMed”. Sono stati presi in considerazione gli articoli pubblicati negli ultimi quattro anni. Gli articoli selezionati sono stati analizzati in base alla loro rilevanza, metodologia di ricerca e risultati. Risultati: Le evidenze mostrano che l’uso di app mobili per il promemoria terapeutico migliora significativamente l’aderenza alla terapia, riduce gli errori di assunzione e favorisce il monitoraggio dei parametri vitali. Inoltre, queste tecnologie promuovono l’autonomia del paziente e possono essere integrate efficacemente nel processo di dimissione ospedaliera attraverso interventi educativi e aggiornamenti delle checklist di dimissione. Conclusioni: Le applicazioni mobili rappresentano uno strumento promettente per supportare i pazienti nella gestione della terapia post-dimissione. L’integrazione di queste tecnologie nei protocolli di dimissione, accompagnata da interventi educativi mirati, può contribuire a migliorare l’aderenza terapeutica e a ridurre i rischi associati alla gestione autonoma della salute.
