Background: Physical activity during pregnancy represents an important factor in promoting maternal health and psychophysical well-being. In particular, water-based exercise offers numerous physical, emotional, and psychological benefits, while also contributing to the reduction of certain obstetric complications. Despite these well-documented advantages, aquatic exercise during pregnancy remains an activity that is still rarely included in antenatal education programs within the Vicenza area. Aim of the research: The aim of this research is to explore and analyze the experiences and perceptions of women who participated in an aquatic exercise program during pregnancy. In addition, midwives working in hospitals and local family counseling centers were interviewed to gather their opinions regarding this activity and to assess their willingness to conduct this type of courses, after an appropriate training, in order to evaluate the opportunity of implementing this practice within the public healthcare system. Materials and methods: Data collection was carried out through three specifically designed questionnaires. Specifically: one questionnaire in Italian language administered to pregnant participants (sample of 70 women), one in English language (sample of 7 women), and a third addressed to midwives (11 professionals working in family counseling centers or hospital settings). Results: The findings outline an overall positive picture. The participants reported physical, emotional, and psychological benefits, as well as greater body awareness and improved preparation for labor and childbirth. Aquatic activity was also perceived as an opportunity for social interaction with other expectant mothers. Most participants expressed willingness to recommend the experience to other pregnant women and to repeat it in future pregnancies. The interviewed midwives shared a positive opinion, recognizing the preventive, therapeutic, and educational value of the program, and expressed readiness to incorporate it into public maternity care services. Conclusions: The survey proves the importance and the need to integrate aquatics exercise programs into birth preparation classes offered by family counseling centers and hospitals facilities, with prior training for the involved midwives, as a valuable tool for promoting maternal well-being, in line with WHO recommendations. Further research on larger samples is recommended to strengthen the current evidence base and to support the development of specific clinical guidelines, as well as the ongoing collection of feedback, which is useful for continuous quality improvement.
