Background: Breastfeeding is a fundamental experience in motherhood, with well-established benefits for both mother and infant. However, the breastfeeding journey can be challenged by physical, emotional, and social difficulties, which are particularly significant in the presence of psychosocial vulnerabilities. In such contexts, professional support, especially from midwives, plays a crucial role. Aim of the study: This thesis aims to investigate whether and how psychosocial vulnerabilities influence the experience of breastfeeding and to assess the perceived effectiveness of midwifery support among mothers in vulnerable situations. Participants: A total of 143 mothers who had given birth at least four weeks earlier participated in the data collection. 28% of the sample reported a condition of psychosocial vulnerability, including psychological or psychiatric disorders, socio-economic hardship, adolescent motherhood, substance use, and experiences of gender-based violence. Materials and methods: This work is based on a descriptive, cross-sectional analysis carried out through the administration of a digital questionnaire, administered between May and August 2025. The questionnaire was inspired by validated tools and included sections on socio-demographic data, infant feeding practices, psychosocial vulnerability conditions, and midwifery support. Findings: Mothers with psychosocial vulnerabilities reported more emotional and relational difficulties related to breastfeeding, although no significant difference was observed in the overall duration of breastfeeding. These mothers perceived midwifery support as less adequate and less personalized, highlighting the need for more sensitive and tailored care. Conclusions: Psychosocial vulnerabilities mainly affect the quality of the breastfeeding experience and related difficulties, without directly impacting its overall duration. The role of the midwife emerges as crucial in providing timely, continuous, and personalized support, integrated within a multidisciplinary model, to promote maternal and infant well-being even in the most vulnerable situations. In particular, the analysis focuses on: The impact of psychosocial vulnerabilities on breastfeeding, considering factors such as mental health, addictions, gender-based violence, and other social and psychological difficulties. The role of the midwife in supporting mothers experiencing these vulnerabilities, with a focus on the practices, strategies, and interventions that midwives can adopt to facilitate breastfeeding and address such challenges.
Introduzione: L’allattamento al seno è un’esperienza fondamentale nella maternità, con benefici riconosciuti per la salute di madre e neonato. Tuttavia il percorso di allattamento può essere ostacolato da difficoltà fisiche, emotive e sociali, particolarmente rilevanti in presenza di fragilità psicosociali. In questi contesti, il supporto professionale, in particolare quello ostetrico, assume un ruolo cruciale. Obiettivo: La tesi vuole indagare se e in che modo le fragilità psicosociali influenzino l’esperienza dell’allattamento materno e valutare l’efficacia del supporto ostetrico percepito dalle madri in situazioni di vulnerabilità. Partecipanti: L’indagine ha coinvolto 143 madri che avevano partorito da almeno quattro settimane. Il 28% del campione ha riferito una condizione di fragilità psicosociale, tra cui: disturbi psicologici e psichiatrici, disagio socio economico, maternità adolescenziale, dipendenze da sostanze e violenza di genere. Materiali e metodi: Il lavoro è di tipo osservazionale descrittivo trasversale ed è stato condotto tramite un questionario somministrato in formato digitale nel periodo di maggio-agosto 2025. Il questionario è ispirato a strumenti validati e comprendeva sezioni su dati socio demografici, modalità di allattamento, condizioni di fragilità psicosociale e supporto ostetrico. Risultati: Le madri con fragilità psicosociali hanno riportato maggiori difficoltà emotive e relazionali legate all’allattamento, pur non mostrando una riduzione significativa nella durata complessiva dello stesso. È emersa una percezione di supporto ostetrico meno adeguato e meno personalizzato in questi contesti, evidenziando il bisogno di un’assistenza più sensibile e mirata. Conclusioni: Le fragilità psicosociali influenzano principalmente la qualità dell’esperienza di allattamento e le difficoltà ad esso correlate, senza incidere direttamente sulla sua durata. Il ruolo dell’ostetrica emerge come cruciale nel fornire un supporto tempestivo, continuo e personalizzato, integrato in un modello multidisciplinare, per promuovere il benessere materno-infantile anche nelle situazioni di maggiore vulnerabilità.
