Background The prodromal phase of labor represents a delicate and complex moment for both women and healthcare professionals. Irregular contractions and variable symptoms make it difficult for many pregnant women to distinguish the onset of active labor, often leading them to present to the emergency department without a clear clinical indication. This results in an increase in inappropriate admissions, causing ward overcrowding, higher healthcare costs, and experiences of anxiety and frustration for the woman. Telemedicine, which has already been successfully applied in the management of certain pregnancy-related conditions such as gestational diabetes, could provide innovative support in this phase by ensuring remote consultation and monitoring. Objectives The aim of this thesis is to explore, through a review of the scientific literature, the role of telemedicine in the management of prodromal labor, considering its clinical, organizational, and experiential impact. In particular, the study seeks to investigate whether teleconsultation can help reduce inappropriate admissions to obstetric emergency departments and improve the quality of care as perceived by women. Materials and Methods A literature review was conducted by consulting major databases (PubMed, Scopus) using targeted search strings to identify studies published over the last ten years. Articles written in English, Italian, and Spanish that addressed the use of telemedicine in prodromal labor were included, while those focusing solely on pregnancy-related comorbidities or on active labor were excluded. The selected studies were analyzed according to the PICO model, assessing population, intervention, comparison, and outcomes. Expected Results The analysis is expected to highlight how telemedicine may help reduce unnecessary visits and hospitalizations, improve maternal well-being and the sense of safety, while also identifying potential organizational challenges related to its implementation.
Background La fase prodromica del travaglio rappresenta un momento delicato e complesso da gestire sia per le donne sia per i professionisti sanitari. Le contrazioni irregolari e i sintomi variabili che la caratterizzano rendono difficile per molte gestanti distinguere l’inizio del travaglio attivo, portando spesso a recarsi in pronto soccorso anche senza una reale indicazione clinica. Questo fenomeno comporta un incremento degli accessi impropri, con conseguente sovraccarico dei reparti, aumento dei costi e vissuti di ansia e frustrazione per la donna. La telemedicina, già sperimentata con buoni risultati nella gestione di alcune patologie della gravidanza come il diabete gestazionale, potrebbe offrire un supporto innovativo anche in questa fase, garantendo consulenza e monitoraggio a distanza. Obiettivi L’obiettivo di questa tesi è esplorare, attraverso la letteratura scientifica, il ruolo della telemedicina nella gestione dei prodromi di travaglio, considerando l’impatto in termini clinici, organizzativi ed esperienziali. In particolare, si cercherà di capire se il teleconsulto può aiutare a ridurre gli accessi impropri al pronto soccorso ostetrico e migliorare l’assistenza percepita dalle donne. Materiali e metodi È stata condotta una revisione della letteratura consultando i principali database (PubMed, Scopus) con stringhe di ricerca mirate a studi pubblicati negli ultimi dieci anni. Sono stati inclusi articoli in inglese, italiano e spagnolo che trattano l’uso della telemedicina nei prodromi di travaglio, mentre sono stati esclusi quelli relativi a patologie gravidiche o al solo travaglio attivo. Gli articoli selezionati sono stati analizzati secondo il modello PICO, valutando popolazione, intervento, confronto e outcome. Risultati attesi Dall’analisi si prevede di mettere in luce come la telemedicina possa aiutare a ridurre accessi e ricoveri non necessari, migliorare la sensazione di sicurezza e il benessere materno e al tempo stesso evidenziare possibili criticità organizzative legate alla sua implementazione.
