This thesis analyzes the discipline regulating the functioning of the National Health Service (SSN), with particular reference to the legislation concerning Essential Levels of Care (LEA), tariff nomenclators and the phenomenon of interregional healthcare mobility. The structure of the SSN, its fundamental and organizational principles, as well as its historical evolution and the role of the National Health Fund are explored in depth (Chapter 1). An in-depth study is then carried out on LEA, from their definition to the updating and monitoring mechanisms (Chapter 2). Subsequently, the topic of tariff nomenclators is addressed, illustrating their regulatory evolution, the function within the SSN and the structure of tariffs for outpatient activities and prosthetic care, with reference also to digital tools for the management of health services (Chapter 3). Finally, the phenomenon of interregional mobility is analysed: its types (active and passive), the mechanisms of economic compensation between the various regions are described and the performance indicators used to assess the efficiency of regional health systems are introduced (Chapter 4). The thesis concludes with some critical reflections and with the exposition of some future perspectives on improving the equity, efficiency and quality of health care in Italy (Chapter 5).
Il presente elaborato analizza la disciplina che regola il funzionamento del Sistema Sanitario Nazionale (SSN), con particolare riferimento alla normativa riguardante i Livelli Essenziali di Assistenza (LEA), i nomenclatori tariffari e il fenomeno della mobilità sanitaria interregionale. Viene approfondita la struttura del SSN, i suoi principi fondamentali e organizzativi, nonché la sua evoluzione storica e il ruolo del Fondo Sanitario Nazionale (Capitolo 1). Si fa poi un approfondimento sui LEA, dalla loro definizione ai meccanismi di aggiornamento e monitoraggio (Capitolo 2). Successivamente, si affronta il tema dei nomenclatori tariffari, illustrandone l’evoluzione normativa, la funzione all’interno del SSN e la struttura delle tariffe per attività ambulatoriali e assistenza protesica, con riferimento anche a strumenti digitali per la gestione delle prestazioni sanitarie (Capitolo 3). Infine, viene analizzato il fenomeno della mobilità interregionale: si descrivono le sue tipologie (attiva e passiva), i meccanismi di compensazione economica tra le varie regioni e si introducono gli indicatori di performance utilizzati per valutare l’efficienza dei sistemi sanitari regionali (Capitolo 4). L’elaborato si conclude con alcune riflessioni critiche e con l’esposizione di alcune prospettive future sul miglioramento dell’equità, dell’efficienza e della qualità dell’assistenza sanitaria in Italia (Capitolo 5).
