ABSTRACT Introduction: The management of psychiatric patients who have committed criminal offenses represents a complex clinical and social challenge, in which the therapeutic approach must integrate healthcare, rehabilitative, and relational competencies. The introduction of REMS (Residences for the Execution of Security Measures) in Italy marked a radical shift in the management of this population, replacing a custodial logic with a therapeutic one. In this context, the nurse’s role becomes central, both in clinical care and in establishing a therapeutic relationship and preventing risk situations. Aim: The aim of this thesis is to analyze the role of nurses in the care of forensic psychiatric patients, highlighting the importance of the therapeutic relationship, multidisciplinary team collaboration, and, above all, overcoming stigma within the rehabilitation and social reintegration process. Materials and Methods: A qualitative descriptive methodology was adopted, based on a narrative review of recent national and international scientific literature. Peer-reviewed articles were selected from medical and scientific databases (e.g., PubMed), focusing on the nursing role in forensic settings, psychiatric rehabilitation, and stigma in mental health contexts. Discussion and Results: The findings emphasize the importance of the nursing role not only in addressing patients’ healthcare needs but also in strengthening the therapeutic alliance, managing dysfunctional behaviors, and promoting a safe relational environment. Overcoming stigma both in society and among healthcare professionals emerges as a crucial factor for ensuring effective care pathways. Furthermore, multidisciplinary team collaboration enhances both the effectiveness of interventions and the well-being of professionals. Conclusion: The nursing profession proves to be central in the care and rehabilitation process of forensic psychiatric patients. Investing in education, interprofessional communication, and the personalization of interventions appears essential to improving clinical outcomes and promoting the genuine social inclusion of individuals.
RIASSUNTO Introduzione: La gestione del paziente psichiatrico autore di reato rappresenta una sfida clinica e sociale complessa, in cui l’approccio terapeutico deve integrare competenze sanitarie, riabilitative e relazionali. L’introduzione delle REMS in Italia ha sancito un cambiamento radicale nella gestione di questa utenza, sostituendo la logica custodiale con quella della cura. Il ruolo dell’infermiere in questo contesto assume una rilevanza centrale, sia nella presa in carico clinica che nella relazione terapeutica e nella prevenzione del rischio. Scopo: L’obiettivo della tesi è analizzare il ruolo dell’infermiere nella presa in carico del paziente psichiatrico forense, evidenziando l’importanza della relazione di cura, della collaborazione in équipe multidisciplinare e, soprattutto, del superamento dello stigma nel percorso riabilitativo e di reinserimento sociale. Materiali e metodi: La metodologia adottata è di tipo qualitativo descrittivo, basata su una revisione narrativa della letteratura scientifica nazionale e internazionale degli ultimi anni. Sono stati selezionati articoli peer-reviewed presenti su banche dati medico-scientifiche (es. PubMed) focalizzati sul ruolo infermieristico in ambito forense, riabilitazione psichiatrica e stigma nei contesti di salute mentale. Discussione e risultati: I risultati emersi evidenziano l’importanza del ruolo infermieristico non solo nella gestione dei bisogni sanitari del paziente, ma anche nel rafforzare l’alleanza terapeutica, nel contenere comportamenti disfunzionali e nel promuovere un ambiente relazionale sicuro. Il superamento dello stigma, nella società ma anche tra i professionisti, risulta fondamentale per garantire percorsi di cura efficaci. Inoltre, la collaborazione in équipe multidisciplinari migliora l’efficacia dell’intervento e il benessere degli operatori. Conclusione: La figura infermieristica si conferma centrale nel processo di cura e riabilitazione del paziente psichiatrico forense. Investire nella formazione, nella comunicazione interprofessionale e nella personalizzazione degli interventi appare essenziale per migliorare gli esiti clinici e promuovere una reale inclusione sociale della persona.
