Background: Fixed orthodontic therapy, either labial or lingual, represents one of the main treatment options for malocclusions. Nevertheless, the placement of brackets, archwires and auxiliaries alters the oral environment, facilitating bacterial plaque build-up and potentially resulting in periodontal complications. Aim of the study: The purpose of this thesis was to compare, based on the available scientific literature, bacterial plaque build-up and periodontal parameters in patients undergoing treatment with labial and lingual fixed orthodontic appliances, in order to identify any clinically relevant differences. Materials and methods: A literature review was conduced using electronic databases (PubMed, Google Scholar) and peer-reviewed dental journals (Journal of Medical Biomedical and Applied Sciences and Revista de Odontologia da UNESP). Clinical studies, systematic reviews, and Randomized Controlled Trials (RCTs) investigating plaque build-up and periodontal status in patients treated with lingual and labial appliances were included. Results: The analysis of the literature indicates that both orthodontic systems promote plaque build-up and an increase in gingival indices in the initial months of treatment. Some studies reported significantly higher plaque scores and Streptococcus mutans colonization in patients with lingual appliances, whereas others found comparable outcomes between the two techniques. Overall, the evidence suggests that these changes are reversible and can be effectively managed through improved oral hygiene and professional care. Conclusions: Labial and lingual fixed orthodontic appliances exert a similar impact on periodontal health, although lingual therapy appears to be associated with a higher risk of plaque accumulation. Comprehensive oral hygiene instruction and regular professional follow-up visits are essential to minimize complications and preserve periodontal health throughout orthodontic treatment.
Background: L’ortodonzia fissa, sia vestibolare che linguale, rappresenta una delle principali opzioni terapeutiche per il trattamento delle malocclusioni. Tuttavia l’applicazione di brackets, archi e accessori ortodontici modifica l’ambiente orale, favorendo l’accumulo di placca batterica e la possibile insorgenza di complicanze parodontali. Scopo del lavoro: L’obbiettivo di questa tesi è quello di confrontare, sulla base della letteratura scientifica disponibile, l’accumulo di placca batterica e i paramenti parodontali dei pazienti trattati con apparecchi ortodontici fissi vestibolari e linguali, al fine di valutare eventuali differenze cliniche rilevanti. Materiali e metodi: È stata condotta una revisione della letteratura consultando database elettronici (Pubmed, Google Scholar) e riviste di pubblicazioni odontoiatriche (“Journal of Medical Biomedical and Applied Sciences” e “Revista de Odontologia da UNESP”). Sono stati inclusi studi clinici, revisioni sistematiche e trial randomizzati. Risultati: Dall’analisi degli studi emerge che entrambi i sistemi ortodontici favoriscono l’accumulo di placca e l’aumento degli indici gengivali nei primi mesi di trattamento. Alcuni autori riportano un aumento marcato degli indici di placca e della presenza di Streptococcus mutans nei pazienti con apparecchi linguali. La letteratura presa in esame concorda nell’affermare che i cambiamenti osservati risultano reversibili e controllabili attraverso il miglioramento dell’igiene orale e il monitoraggio professionale. Conclusioni: Entrambi i tipi di ortodonzia fissa hanno un impatto simile sulla salute parodontale, con un maggior rischio di accumulo di placca nella terapia linguale. L’educazione all’igiene orale domiciliare e i richiami periodici di follow-up sono determinanti per prevenire complicanze.
