Introduction: The role of sports physiotherapists has become increasingly relevant over time in the management and prevention of injuries, contributing to athletes’ functional recovery and return to play. Although international literature has addressed the relationship between expectations and adherence to sports rehabilitation, specific studies in Italy remain limited. In amateur settings, the physiotherapist’s presence is often irregular, affecting both the quality of the therapeutic relationship and the continuity of treatment. Objectives: This study aimed to explore expectations and adherence to rehabilitation treatment within an amateur football team, examining the relationship between relational factors (therapeutic alliance) and behavioral factors (adherence to the rehabilitation program). Materials and Methods: Fourteen athletes (mean age = 24.1 ± 4 years) participated in the study. Two instruments were administered: the Exercise Adherence Rating Scale (EARS) to measure treatment adherence and the Expectation About Athletic Training (EAAT) to assess expectations toward physiotherapy. Data were analyzed using descriptive statistics, correlation analyses, and cluster analysis. Results: The mean EARS score was 14.4 ± 4.4, with most athletes showing moderate adherence levels. The highest EAAT scores were observed in the dimensions of directiveness, genuineness, and immediacy, highlighting the importance attributed by athletes to professional competence and effective communication. A positive and significant correlation between expectations and adherence was found (ρ=0.624; p=0.017), revealing three distinct motivational profiles among the athletes. Conclusions: Positive expectations toward the physiotherapist are associated with greater adherence to rehabilitation programs. Building a solid therapeutic alliance, based on trust, clear communication, and perceived competence, plays a crucial role in optimizing clinical outcomes and facilitating athletes’ return to play, particularly in amateur sports contexts.
