INTRODUCTION: The therapeutic relationship is a central aspect of physiotherapy, as it can influence clinical and psychosocial outcomes as well as patient engagement. In pediatrics, it is more complex, requiring the physiotherapist to adapt the approach to the child’s developmental and emotional characteristics, fostering collaboration through empathy, play, and family involvement. However, research on the role of relational skills in this context remains limited. OBJECTIVE: This qualitative study aims to explore and highlight the role of the therapeutic relationship between physiotherapist and patient in the pediatric context, emphasizing its influence on patient engagement and treatment effectiveness. MATERIALS AND METHODS: A qualitative study was conducted through the administration of a questionnaire to 30 physiotherapists working with children. The construction of the questionnaire involved an initial literature review using Google Scholar and the scientific database PubMed with the keywords “Therapeutic Relationship,” “Physical Therapy,” “Children,” “Physiotherapy.” Subsequently, five main areas of analysis were defined, leading to the formulation of 22 questions. The responses provided by the physiotherapists were recorded and processed, followed by qualitative analysis and interpretation of the data, which were then integrated with the theoretical framework provided by the scientific literature. RESULTS: The responses collected from physiotherapists made it possible to investigate the concept of the therapeutic relationship in relation to empathy, family involvement, efficacy factors, and challenges. Regarding psychological and communication aspects, nonverbal communication, relational strategies, and training were explored. With respect to professional ethics, the management of complex situations and the importance of the team’s role were analyzed. Finally, personal reflections and suggestions from the therapists were gathered, offering insights for improving the therapeutic relationship between therapist and patient in pediatric settings. The analysis of the results confirmed the importance of these elements in therapeutic relationships. CONCLUSION: This study highlights that therapeutic relationship, based on empathy, effective communication, and child and parental involvement, represents an essential element in pediatric physiotherapy, supported by technical skills, multidisciplinary teamwork, and continuous professional development. These factors ensure a rehabilitation process based on child- and family-centered care.
