Parietal stroke can lead to different cognitive impairments depending on the location and lateralization of the lesion. Among these, acalculia and finger agnosia represent two neurologically associated deficits since they can manifest together, as classically described within Gerstmann syndrome. Although this syndrome, and consequently the deficits, is typically associated with damage to the left hemisphere, recent evidence challenges the traditional view of a sole left-hemisphere specialization, suggesting that numerical cognition relies on a bilateral frontoparietal network. This thesis investigated the relationship between acalculia and finger agnosia in patients with unilateral parietal stroke. The study aimed at examining the occurrence and co-occurrence of the deficits across the left and right hemispheres, expecting a wider association in the left. The first part of the thesis provides a general overview of the anatomy and functions of the parietal lobe, along with the most known neuropsychological deficits associated with parietal damage. The second part examines the results of patients who completed the NADL test and the Finger Recognition test, using both qualitative and quantitative analyses. Contrary to the expectations, the findings indicated no statistically significant association between the two tests’ results within each hemisphere, although a low-to-moderate effect size was observed in patients with right-hemisphere damage. Further analyses are needed to clarify whether the result is due to the small sample size as well as other confounding deficits.
L’ictus parietale può provocare una vasta gamma di deficit a livello cognitivo, a seconda dell’area interessata e dell’estensione della lesione. Tra questi, acalculia e agnosia digitale rappresentano due deficit connessi dal punto di vista neurologico, in quanto possono manifestarsi in concomitanza, come descritto nell’ambito della sindrome di Gerstmann. Sebbene tale sindrome, e di conseguenza i suddetti deficit, siano tipicamente associati a lesioni dell’emisfero cerebrale sinistro, recenti studi contestano l’idea che le abilità di cognizione numerica siano solamente attribuibili a tale emisfero, suggerendo per contro una possibile specializzazione bilaterale dei circuiti fronto-parietali. Il presente elaborato verte sulla relazione tra acalculia e agnosia digitale in pazienti colpiti da ictus parietale unilaterale. Lo studio è volto ad esaminare l’occorrenza e la co-occorrenza di tali deficit nei due emisferi cerebrali, con l’ipotesi di una maggiore associazione a sinistra. La prima sezione dell’elaborato delinea un quadro d’insieme relativo all’anatomia e alle funzioni associate al lobo parietale, illustrandone i principali deficit conseguenti a ictus. La seconda sezione è dedicata alla valutazione di dati clinici di una coorte di pazienti, esaminata mediante il test NADL (Numerical Activities of Daily Living) e il test di Riconoscimento delle dita, attraverso un approccio di analisi qualitativa e quantitativa. Contrariamente a quanto previsto, i risultati non hanno evidenziato alcuna associazione statisticamente significativa tra i punteggi dei test all’interno dei gruppi divisi per emisfero cerebrale, sebbene sia stata osservata una certa dimensione dell’effetto per quanto riguarda i pazienti con lesione destra. Saranno necessari ulteriori approfondimenti per chiarire se tale esito sia dovuto alla ridotta numerosità campionaria nonché alla presenza di eventuali fattori di confondimento.
