Thrombin (αT) is a serine protease that plays a dual role in regulating coagulation homeostasis: on the one hand, it catalyzes all the reactions necessary for proper coagulation, on the other, it ensures the maintenance of blood fluidity in regions unaffected by the lesion. Prethrombin-2 (Pre2) is the immediate zymogen of αT and represents a widely used model for studying the relationships between the structure, conformation dynamics, and catalytic function of αT. Recently, a new thrombin variant called Padova 3 (Glu77Lys; ProT-PD3) was identified in a patient admitted to the University Hospital of Padua following recurrent thrombotic events. The Glu77Lys mutation involves loop 70, a region that, together with loop 30, contributes to the formation of exosite I of αT. This region is responsible for recognizing both procoagulant substrates (fibrinogen and PAR1) and anticoagulant ones (thrombomodulin). This thesis investigated the structural and conformational basis of the altered procoagulant function of the PD3 variant by studying Pre2-PD3 and αT-PD3, and comparing them with the respective Pre2-WT and αT-WT forms. The Glu77Lys variant was produced by site-directed mutagenesis on the plasmid encoding the wild-type Pre2 protein and expressed in E. coli cells. It was subsequently purified by biospecific affinity chromatography, and its molecular identity was determined by SDS-PAGE, RP-HPLC, and high-resolution mass spectrometry. The αT form was obtained by proteolytic activation with Echis carinatus II protease. Circular dichroism and fluorescence spectroscopy analyses have shown that the Glu77Lys substitution causes a perturbation in the conformational structure of the Pre2 species. Hydrogen-Deuterium Exchange Mass Spectrometry (HDX-MS) analyses of the inactive forms confirm and complement the findings from biospectroscopic analyses, indicating a marked increase in deuterium incorporation for the mutated protein. The same behavior was observed in global HDX-MS analyses of the αT-WT and αT-PD3 forms. Local HDX-MS analyses of the inactive forms also confirm the perturbation and increased flexibility observed in all previous analyses. Local HDX-MS analyses of the αT-WT and αT-PD3 forms identify three peptides as primarily responsible for the increased degree of disorder in αT-PD3 detected by global HDX-MS analyses. These peptides include loop 70, loop 30, and the catalytic histidine (H57). Overall, the results demonstrate that the Glu77Lys substitution induces significant alterations in the protein's structural and dynamic properties, shifting the conformational equilibrium toward more flexible states compared to the wild-type form and disrupting both the structure of exosite I and the protein's flexibility around the catalytic histidine. These modifications could contribute to the molecular mechanisms underlying the unique thrombotic phenotype associated with the Padova 3 variant.
