Narrative review. Background: Adhesive capsulitis is an inflammatory disorder that causes loss of mobility in the scapulohumeral joint. The onset is more frequent in women, between the ages of 35 and 50 years and is often associated with metabolic diseases (diabetes or hyper/hypothyroidism) and autoimmune problems. Treatment is focused on pain reduction and recovery of joint function; some therapeutic strategies involve drug therapy combined with physiotherapy treatment, corticosteroid injections and, if necessary, surgery performed arthroscopically. Aims: To describe the role of re-education of scapular kinematics in reducing symptoms and improving recovery of shoulder articulation in patients with adhesive capsulitis. Materials and methods: This study is a narrative review of the literature; the literature search was conducted on the online databases PubMed, PEDro and The Cochrane Library, without placing limits on the language of publication or gender of the participants but with a minimum limit on the age of the participants (at least 18 years) and the year of publication (after 2015). Some additional researches were conducted using the bibliography of systematic reviews related to the topic. The outcomes considered were: Joint ROM (active and passive), Shoulder Pain and Disability Index (SPADI), Disability of Arm Shoulder Hand score (DASH), Visual Analog Scale (VAS). Results: Out of the 375 articles identified through the databases, 8 studies were included in the review. In the analysed studies, the re-education of scapular kinematics had a significant effect for all analysed outcomes. Conclusions: In the present review, despite the variability of the studies analysed, an increase in the joint ROM values, SPADI index, DASH and VAS scores, with a consequent improvement in the function of the upper limb, was shown. With these data it’s possible to conclude that drawing up and applying a rehabilitation programme that also includes the observation and re-education of scapular kinematics is effective in the rehabilitation of adhesive capsulitis, especially in the long term.
Revisione narrativa. Background: la capsulite adesiva è una patologia infiammatoria che causa la perdita di mobilità dell'articolazione scapolo omerale. L’esordio è più frequente nel sesso femminile, in un'età compresa fra i 35 e i 50 anni e si associa spesso a malattie metaboliche (diabete o iper/ipotiroidismo) e a problemi autoimmuni. Il trattamento è focalizzato sulla riduzione del dolore e sul recupero della funzionalità dell’articolazione della spalla; alcune strategie terapeutiche riguardano la terapia farmacologica associata a trattamento fisioterapico, iniezioni di corticosteroidi e, se necessario, intervento chirurgico effettuato in artroscopia. Obiettivi: descrivere il ruolo della rieducazione della cinematica scapolare nella riduzione dei sintomi e nel miglioramento del recupero dell’articolarità della spalla nei pazienti affetti da capsulite adesiva. Materiali e metodi: questo studio è una revisione narrativa della letteratura; la ricerca bibliografica è stata condotta sulle banche dati online PubMed, PEDro e The Cochrane Library, senza porre limiti di lingua di pubblicazione o sesso dei partecipanti ma con limite minimo di età dei partecipanti (almeno 18 anni) e di anno di pubblicazione (successivi al 2015). Alcune ricerche aggiuntive sono state condotte utilizzando la bibliografia di revisioni sistematiche correlate all’argomento trattato. Gli outcome considerati sono stati: ROM articolare (attivo e passivo), Shoulder Pain and Disability Index (SPADI), Disability of Arm Shoulder Hand score (DASH), Visual Analog Scale (VAS). Risultati: dei 375 articoli identificati attraverso i database sono stati inclusi nella revisione 8 studi. Negli studi analizzati, la rieducazione della cinematica scapolare ha avuto un effetto significativo per tutti gli outcome analizzati. Conclusioni: nella presente revisione, nonostante l’eterogeneità degli studi analizzati, è stato evidenziato un aumento dei parametri ROM articolare, indice SPADI, punteggi DASH e VAS, con un conseguente miglioramento della funzionalità dell’arto superiore. Con i dati emersi è possibile concludere che redigere ed applicare un programma riabilitativo che includa anche l’osservazione e la rieducazione della cinematica scapolare sia efficace nella riabilitazione della capsulite adesiva, soprattutto nel lungo termine.
