Patients with disorders of consciousness are increasingly in number thanks to medical advances in allowing survival after severe brain trauma. It is a challenge for medical personnel to assess the severity of the injury and for physiotherapists to set up rehabilitation interventions. Spasticity and pain are the most frequent problems, but the real difficulty lies in understanding whether interventions (pharmacological or not) are effective in improving their condition and state of well-being, even more so when dealing with patients in prolonged state of altered consciousness. Physiotherapists have limited tools to intervene on their quality of life, so it is necessary to evaluate the state of well-being more carefully to establish the effectiveness of the treatment. A bibliographic search on PubMed and PEDro was conducted in order to investigate the tools available to evaluate the state of pain and set the best physiotherapy treatment in order to improve the state of well-being in these patients.
I pazienti con disordini di coscienza sono sempre più numerosi grazie ai progressi della medicina nel consentire la sopravvivenza dopo gravi traumi cerebrali. È una sfida per il personale medico stabilire la gravità del danno e per i fisioterapisti impostare interventi riabilitativi. La spasticità e il dolore sono le problematiche più frequenti, ma la vera difficoltà sta nel comprendere se gli interventi (farmacologici e non) sono efficaci nel migliorare la loro condizione e lo stato di benessere, ancora di più se si tratta di pazienti in prolungati stati di alterazione della coscienza. I fisioterapisti hanno strumenti limitati per intervenire sulla loro qualità di vita, per cui si ha necessità di valutare lo stato di benessere in maniera più oculata per stabilire l’efficacia del trattamento. È stata condotta una ricerca bibliografica su PubMed e PEDro allo scopo di indagare gli strumenti a disposizione per valutare lo stato di dolore e impostare il miglior trattamento fisioterapico al fine di migliorare lo stato di benessere in questi pazienti.
