Online cognitive testing is emerging as a scalable and accessible alternative to traditional neuropsychological assessment. This review examines its application in neurodegenerative and mental health disorders, including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, schizophrenia, and bipolar disorder. Evidence suggests that digital tools can facilitate early detection, monitoring, and improved access to care, particularly in underserved populations. However, challenges remain, including limited large-scale validation, reliance on self-report measures, and a lack of standardized protocols and normative datasets. Addressing these gaps is essential to ensure scientific rigor and clinical reliability. With further validation, online cognitive testing could become an integral tool in clinical practice.
