The purpose of this thesis is to provide an in-depth study of fructosemia, a rare genetic disease that makes even minimal intake of fructose, sorbitol, and sucrose toxic. The work integrates the scientific and clinical foundations of the disorder, from genetic transmission to metabolism, as well as symptomatology and diagnosis, with an analysis of dietary strategies and nutritional monitoring. Particular attention is devoted to food labeling, with an examination of European and national regulatory references, interpretative ambiguities, and the practical consequences for consumers affected by fructosemia, supported by concrete examples of potentially misleading labels. The thesis also explores the recognition of fructosemia as a rare disease and the limits of healthcare support, highlighting the critical issues of the current regulatory system and the implications on the right to safe nutrition. Finally, the role of the Italian Association of Fructosemic Patients (AIF) is presented, through a reconstruction of its history, activities, and projects, together with a direct testimony that sheds light on the daily difficulties and the often invisible social dimension of the disease.
Lo scopo di questa tesi è approfondire la fruttosemia, una rara malattia genetica che rende tossica anche una minima assunzione di fruttosio, sorbitolo e saccarosio. L’elaborato integra i fondamenti scientifici e clinici della patologia, dalla trasmissione genetica al metabolismo, fino alla sintomatologia e alla diagnosi, con l’analisi delle strategie dietoterapiche e del monitoraggio nutrizionale. Un’attenzione particolare è rivolta all’etichettatura alimentare, con l’analisi dei riferimenti normativi europei e nazionali, delle ambiguità interpretative e delle conseguenze pratiche per i consumatori affetti da fruttosemia, supportata da esempi concreti di etichette potenzialmente fuorvianti. La tesi approfondisce inoltre il riconoscimento della fruttosemia come malattia rara e i limiti dell’assistenza, mettendo in luce le criticità dell’attuale sistema regolatorio e le implicazioni sul diritto a un’alimentazione sicura. Infine, viene presentato il ruolo dell’Associazione Italiana Fruttosemici (AIF), attraverso la ricostruzione della sua storia, delle attività e dei progetti, con una testimonianza diretta che evidenzia le difficoltà quotidiane e la dimensione sociale, spesso invisibile, della malattia.
